About the Role:

The Fono is offering an exciting opportunity for a Registered Nurse who is dedicated to improving Pacific Health. In this role, you will work closely with the general practice team and social workers to address the healthcare needs of patients facing barriers to accessing services. The 'RN Outreach' position involves delivering vital medical services to vulnerable patients in their homes, supporting them on their journey to better health.

As part of The Fono's integrated model of care "FonoCare," Primary Health Outreach RNs collaborate with GPs, HCAs, social workers, mental health specialists, and various other healthcare professionals to uplift the well-being of Pacific families.

About You:

If you have experience in primary health or community care, excel in chronic care management, and are committed to Pacific approaches to healthcare, you are the ideal candidate for this role. Holding NZ citizenship or permanent residency is a requirement. This position provides an opportunity to grow within an organization that is truly making a difference in the community.
