Position Details

In this role, you will be responsible for providing psychosocial assessment, support, and information to individuals and families affected by sickness. Your duties will include conducting home visits, formulating care plans, providing counseling on social and financial impacts, and facilitating access to external resources and services.

Requirements

• Current APC as a social worker
• Experience in physical health conditions
• Ability to work independently and as part of a team
• Full NZ Driver's License
