Job requirements:

- Health Coach experience and training or specialist diabetes training - Understanding of health, social services, and mental health challenges within Pacific communities - Knowledge of protocols amongst Pacific peoples with the ability to work across various Pacific groups - Positive attitude and comfort in a developing service setting

Job responsibilities:

- Support patients and families in achieving wellness goals - Focus on diabetes reversal/remission/good management and prevention - Provide emotional support and link families to relevant services - Work collaboratively with a multidisciplinary team

Qualifications:

- Health Coach experience and training or specialist diabetes training

Skills:

- Understanding of health, social services, and mental health challenges within Pacific communities - Knowledge of protocols amongst Pacific peoples - Positive attitude and adaptability to a developing service setting
