About the Initiative
ProCare has launched the Comprehensive Primary Care Team (CPCT), a groundbreaking model that aims to enhance health equity by expanding service access through innovative roles within existing general practice teams. This initiative is fully funded until the end of June 2025 and integrates allied health, social work, and pharmacy professionals into practices to better serve at-risk communities. By supporting free patient access to essential services, the initiative eases pressure on GP teams while improving family-focused care on-site.
Current Vacancies
ProCare is currently looking to fill the following part-time positions in various practices:
- South Auckland Mangere practice - 16 hours per week
- Manukau practice - 16 hours per week
- Papakura practice - 32 hours per week
About the Role
The Practice Pharmacist will serve as the subject matter expert within the practice on all matters related to medicines information and advice. Responsibilities include offering individualized pharmacotherapy advice to patients, providing education and pain management sessions, and enhancing awareness of medicine-related harms. Some key duties of the role include:
- Medicines management, optimization, and reconciliation
- Clinical evaluations and effectiveness of best practice
- Medication evaluation, audit, and quality improvement
- Development and management of standards, policies, and procedures
Requirements
ProCare is looking for Pharmacists with a diverse range of experience levels and backgrounds. Ideal candidates should have:
- A current APC with no restrictions
- Post-grad studies in clinical pharmacy or pharmacist prescribing
- 2+ years of experience as a registered pharmacist
- Comprehensive understanding of health inequities
- Strong cultural competency and knowledge of Te Tiriti o Waitangi
