The Role
We are looking for a full-time Health Coach to work closely with patients and their families in our GP clinics, supporting them with long-term health conditions and overall well-being. This role involves collaborating with nurses, GPs, and community support workers to promote holistic well-being in a busy clinic setting. As a Health Coach, you will provide self-management support, continuity of care, and connect patients to necessary resources. Training will be provided, and successful applicants will have the opportunity to deliver evidence-based group programs.
Requirements
- Excellent interpersonal and communication skills
- Cultural and linguistic diversity
- Basic computer skills and eagerness to learn new systems
- Interest in using coaching practices to improve well-being
- Secondary school qualification
- Ability to adhere to evidence-based models
- Experience or interest in delivering group programs
