Job responsibilities

- Work with the general practice team - Support patients/clients and their families - Develop skills, tools, confidence, and lifestyle choices for wellness goals - Provide emotional support and link families to relevant services - Aligned with Pacific models of care

Qualifications

- Broad understanding of health, social services, and mental health challenges in Pacific communities - Ability to speak Tongan is advantageous - Knowledge of protocols among Pacific peoples - Positive attitude in times of stress

Skills

- Understanding of health, mental health, and social services - Ability to speak Tongan is a plus - Cultural competence in working with Pacific communities - Positive attitude and belief in the ability to support patients and families
