About the Role

As a Connector Case Manager at Taikura Trust, you will be responsible for providing quality case management to clients with high and complex needs. Your role involves assessing risks, exploring support options, and connecting clients with the necessary services and communities to achieve their desired outcomes. You will work closely with clients and their families to create personalized engagement models and support plans.

Requirements

  • Minimum of three years of experience in the health, allied health, education, or disability sector
  • Proven track record in coordinating supports for individuals with complex physical, intellectual, and sensory needs
  • Excellent communication and organizational skills with a focus on quality and improvement
  • Commercially savvy with the ability to navigate IT systems
  • Familiarity with Te Tiriti o Waitangi and the ability to apply it in practical situations
