ProCare Fresh Minds is seeking Health Improvement Practitioners to join their team in Papatoetoe, New Zealand.

Role Overview

In this role, you will be providing evidence-based interventions to empower individuals to achieve their health and wellbeing goals through brief interventions. As a Health Improvement Practitioner, you will work within General Practice clinics, focusing on delivering quality care to the community, with a special emphasis on Māori, Pasifika, and youth populations. You will receive specialized training in Primary Care Behavioral Health and Acceptance and Commitment Therapy, allowing you to make a meaningful difference in people's lives.

Responsibilities

  • Providing brief psychological interventions, including Focused Acceptance and Commitment Therapy (FACT)
  • Delivering evidence-based group programs to patients
  • Consulting, educating, and advising other members of the General Practice team
  • Developing intervention pathways alongside GPs and nurses to support local population health

Requirements

To excel in this role, you should be a registered mental health professional with training in talking therapies such as ACT, FACT, or CBT. You should be self-motivated, team-oriented, resilient, and adaptable to the fast-paced environment of a General Practice. Strong IT skills and the ability to work with diverse teams are essential for success in this role.
