The Opportunity:

As a Health Navigator – Kaimanaaki at ProCare, you will have the chance to make a tangible impact in healthcare across New Zealand. Your role will involve supporting whānau in their health and wellbeing journeys, promoting self-management, and connecting them with necessary partner services. You will work closely with multiple general practice teams to reduce barriers to accessing support.

Key Skills/Attributes Required:

• Experience working in healthcare with patients, community groups, and organizations
• Ability to effectively interact with people from diverse cultures
• Experience working with Māori & Pacific Island populations preferred
• Flexibility to travel across multiple clinics within designated regions
• Proficiency in IT and software systems
• Clean NZ driver's license

Preferred Qualifications and Experience:

• Certificate in Health and Wellbeing at Level 3 or above
• Ability to speak basic Te Reo Māori and/or a Pacific language
• Experience in Mental Health and/or addictions services
• Training or experience in Motivational Interviewing, Behavioral change approaches, Smoking Cessation, and/or Alcohol Brief Intervention Support
