• Lead the Health Services Connection Centre team
• Work with clinicians and administrators to improve community health outcomes
• Manage population health registers and care connection services
• Collaborate with various health projects and teams

Skills and Experience

• 5-10 years of senior health services experience
• Experience in people leadership and workload management
• Strong background in life course area
• Ability to build relationships at a senior level
• Experience in developing effective strategies and work plans
