About Whanau Ora:
Whanau Ora is a government initiative focused on strengthening the wellbeing and future of Pacific families. It emphasizes culturally specific approaches that empower families to find solutions to their challenges. At Tongan Health Society, we are dedicated to supporting Pacific families in areas such as education, training, economic development, and health, among others.
Key Responsibilities:
- Identify potential families for the Whanau Ora program
- Complete assessments using the MAST tool
- Facilitate family enrollment and provide support
- Conduct formal assessments to inform family plans
- Support families in implementing their plans
- Monitor progress and report regularly
Essential Skills:
- Experience working with Pacific clients and families
- Understanding of Pacific values and models of care
- Effective communication and engagement skills
- Familiarity with data entry tools and patient management systems
- Excellent case management abilities
- Knowledge of non-clinical navigation pathways
Desirable Skill:
Bilingual in English and Tongan
